Thanks to Zhang for sending in this Los Angeles Lakers Ultimate Ten Patch he recently pulled from his purchase of  2008/09 Upper Deck Ultimate Collection Basketball

The front of the card features jersey patches from five Lakers legends – Wilt Chamberlain, Jerry West, Pat Riley, Magic Johnson, and Michael Cooper – and the back features jersey patches from five Lakers who recently won the NBA Championship – Kobe Bryant, Lamar Odom, Luke Walton, Sasha Vujacic, and Jordan Farmar.

The card is numbered 3/3 and has some awesome patch breaks, check out the fading and color difference on the Jerry West and Pat Riley patches.
